Thomas J. Lewis, Ph.D. – Medical Scientist Chief Health Officer / Founder
• Ph.D. MIT (Chemistry)
• Harvard School of Public Health (Toxicology, Nutrition)
• Trained under two medical thought leaders at Harvard
• Kilmer McCully – Pathologist;
• Clement Trempe Ophthalmologist Accomplishments:
• Six books including a medical text book on Alzheimer’s
• Multiple peer-reviewed papers and treatment patent for Alzheimer’s

Mission: To implement an affordable healthcare system to replace the existing sickcare
system.
To provide doctors and healthcare practitioners tool to evaluate and reverse human diseases regardless of their severity and complexity.
Contributions to Healthcare:
1. The health-disease continuum. No one is either healthy or sick. Instead we all reside somewhere on a continuum which can be determined with precision and accuracy.
2. Disease mechanisms as opposed to syndromes & diagnostic codes.
3. Hierarchy of health. The process of evaluating the interdependency of disease mechanisms at established a prioritized workflow of dependent and independent health variables.
4. Establishment of science-based biomarker reference ranges. This involved curve-fitting hazard risk ratios for early “all-cause” mortality. Each biomarker is assigned a weighted contribution to early mortality. The end product is a single health risk score value easily tracked to evaluate intervention efficacy.
Nathan Rozenfeld – CTO/Senior Enterprise Architect Chief Technology Officer / Founder
• Odessa State Polytechnic University (Master Computer Science)
• Trade Point Israel Ltd
• Unizag Ltd
• StarLIMS
• NHR Inc
• Founder, EPH Solutions Accomplishments: Coordinated NIH initiatives to define and deploy National Healthcare Network;
W.H.O. Epidemiology and Early Response Network; Directed several CDC/APHL/NIH Healthcare IT initiatives Twenty+ years IT career I was focusing on providing practical and working
solutions in many fields of enterprise information management and automation. Such a unique firsthand experience and extensive core-matter knowledge permitted me to obtain a deep level of familiarity with industry operations.
Over the years, I played key roles in various projects in the field of public health at the federal and regional levels, management and control over the processes of design, development and implementation of medical operating platforms as
well as LIMS/LIS (Laboratory Information Management Solutions), Revenue Cycle Management, Compliance as few others.
Some of the projects had national impact and focused to provide cutting edge solutions for CDC-CCID, WHO (World Health Organization) and other federal agencies. During my professional career, I have successfully deployed over 100 Enterprise
Information Management solutions worldwide. Nathan is the chief architect of the S5P Health system.
Problem:
America’s Healthcare System is Failing.
America ranks last in the cost/benefit ratio of all 36 developed nations.
Health Expenditure per capita
Solution: Scalable P5 Medicine
Predictive, Precision, Personalized, Preventative, and Participatory
• Biomarker assessment is the cornerstone of health evaluation.
The standard-of-care references ranges are NOT science based.
We developed an evidence-based reference range algorithm
titrated to early mortality risk that reflects both acute AND chronic
risk. The latter is missed in the standard of care.
• Health/Disease Continuum (HDC). Lab biomarker measurements
provide an objective assign to a person’s position on the
continuum. Robust risk measurements explain the ”why” of the
placement and drives interventions.
• Five (5) Disease Mechanisms. The standard-of-care ICD-10
defines 68,000 diagnostic codes. However, ~5 mechanisms explain
90% of morbidity and mortality. Approaching healthcare
interventions based on these mechanisms affords tremendous
efficiencies and superior outcomes. It is a “root-cause” approach
while emphasizing that most human diseases stem from a limited
set of risk factors.
• Hierarchy of Health (HOH). The HOH establishes a science-based “systems”
approach to reversing health problems. This step-by-step method
acknowledges the interconnectivity of physiological systems. It employs the
concept of dependent and independent variables to continuously reduce
costs, increase efficiencies, while improving outcomes
How it works – for individuals and populations
Interoperative EMR Framework.
• HIPAA compliant
• Push/Pull data from any source
• Complete billing and scheduling
• Clinical notes and reporting
Scalable P5 Framework
• All possible data digitized for mining
• Sophisticated “intake” survey establishes baseline risks
• ”Sticky” actionable risk report
• MI (medical intelligence)-driven workflow intervention
• Any/All labs run through proprietary algorithm – established HDC
• Evidence for all data available to clients
Efficient Health Information Delivery
• Patient provided with detailed / personalized workflow
• Provider consult to answer questions and guide workflow
• Coaches afford low-cost guidance
Scalability & Monetization
• Digitized data enables identification of groups with common risks
and diseases processes.
• Ability to contact “groups” and establish volume consultations.
• Risks and issues identified with precision affords opportunity to
embed the selling of: supplements, pharmaceuticals, systems,
labs, additional services, often without the need for a private
consult.
Here are your positions on key categories of health.
